Student Identification Document:
Parents will be required to upload a document that can be used to verify the student's identity. This document will not be shared with any other entity and will only be used for internal auditing procedures. Acceptable documents include:
- Birth Certificate
- Government ID
- School ID
- Any other document that can be used to verify the student’s identity
Active Duty Military:
Active Duty Military members who are either stationed inside the state or a Utah resident stationed out of state must upload military orders.
Residency Documentation:
Parents must upload documentation to verify their primary residence is the state of Utah. Eligible documents must be dated within the current calendar year and include the name and address provided during the application. Eligible documents are as follows:
- Utility Bill
- Mortgage Statement
- Lease Agreement
- Property Tax Records
- Bill of House Sale
Income Documentation:
This is only for new applicants and returning parents adding a new student. Returning applicants will not need to verify income.
As part of the application process, parents will provide written consent for income verification through the State Tax Commission. To determine funding prioritization, parents must provide information to Odyssey to determine household income. The following is necessary:
- Parent Social Security Number
- Household Size
- Federal 2024 (acceptable for conditional approval) or 2025 1040
The State Tax Commission will verify household income, not individual income, within 10 business days (§53F-6-402(6)(c)-(d)).
Returning Student Portfolio/Assessment:
Portfolio Requirements: To maintain eligibility for the following school year, you must submit one document that describes your child's educational opportunities and achievements under the program for the given year.
Examples of artifacts that will meet the portfolio requirements include:
A short written summary or essay describing your student’s educational opportunities, focus areas, and progress over the year.
Samples of student work, such as writing assignments, projects, lab reports, artwork, or completed curriculum units.
A report card or progress report from a private school or curriculum provider.
Assessment results, including standardized tests, reading level assessments, college readiness exams, or other nationally or state-recognized assessments as described in Section 53F-6-410. Utah state recognized tests and assessments can be found here.
A collection of learning artifacts, such as photos of projects, portfolios of coursework, or summaries of major learning milestones.
