This article is intended to help vendors and service providers understand what documents and information they need to have on hand when applying to join Wyoming ESA's Odyssey Marketplace.
To become a service provider, you must complete the Wyoming ESA vendor application and provide basic business information. As part of the application process, you will need to select your service provider type, each of which has specific requirements. To ensure a smooth application process, gather the necessary documents and information in advance.
Below is a list of the required documents and information based on your provider type:
Schools (K-12 and College / Post-secondary)
- Name of Accreditation Agency
Pre-Kindergarten Program
- DFS Child Care Facility Licensure Number
- A copy of your current certifications, licenses, or specialized training relevant to the services you provide (must be uploaded)
Educational Service Providers
If Applying as a Business:
- Name of Accrediting/Licensing Agency (if applicable)
- FEIN (Federal Employer Identification Number)
- A copy of your Business License or Articles of Incorporation (must be uploaded)
- A copy of your current certifications, licenses, or specialized training relevant to the services you provide (must be uploaded)
If Applying as an Individual:
- ITIN (Individual Taxpayer Identification Number) or Social Security Number
- A copy of your current certifications, licenses, industry certifications for CTE providers, and/or specialized training relevant to the services you provide (must be uploaded)
Vendors (Tangible Goods)
- A copy of your Business License (must be uploaded)
