Please note in order to be eligible for funding, documentation must be uploaded for the following criteria:

| Student has IEP | Upload of IEP document or approved learning plan |
| Student in foster care | Upload of Court or government signed document indicating foster care |
| Parent or guardian in an active duty military role (includes National Guard who is in active military status) | Upload a copy of active duty orders |
| Parent or guardian killed servicing in the line of duty | Upload of DD-1300 for military or attestation from department for first responders |
| Application for “Option Enrollment” has been denied | Upload of official letter or email from the district with denial notice |
| Experience bullying, harassment, hazing, assault, battery, kidnapping, robbery, sexual offenses, threat or intimidation, fighting at school | Upload of documentation or correspondence that a parent or student has communicated the mistreatment to law enforcement, mental health provider, health care provider, public school educator, public school counselor, public school administrator, or other public school staff. Documentation must be no older than one calendar year. |
