Who can be a Utah Fits All Scholarship provider?

To become a Utah Fits All Scholarship provider you must meet the following requirements outlined in Utah Code § 53F-6-409 of HB 455. 

1. Complete the Odyssey Provider application process:
   1. The Provider Application for the Utah Fits All Scholarship Program launched on June 11th, 2025.

   2. Providers under the previous program administrator must reapply.
   3. Providers must agree to Odyssey’s terms and conditions.
   4. Both the provider and each offering they upload will be reviewed for UFA Scholarship Program compliance

2. Provide your basic details:
   1. Give your federal tax ID number (EIN).
   2. List your address and contact information.
   3. Describe the programs or services you’ll offer to scholarship students.
   4. Submit any additional information requested by the program manager.
       
3. A provider must:
   1. Comply with Title VI of the Civil Rights Act (42 U.S.C. § 2000d), which prohibits discrimination based on race, color, or national origin in programs that receive federal funding. Learn more at https://www.govinfo.gov/link/uscode/42/2000d.
   2. Provide services directly—services must be provided by the provider themselves; no middlemen.
   3. Maintain approval status—once approved, the provider’s name appears on a public list; if ownership changes, the provider must reapply and be reapproved.
       

4. A provider cannot be:

   1. A parent of a home-based scholarship student or a home school student solely in relation to the parent’s child
   2. Any other individual that does not meet the requirements described in this section
