**This instruction document is for parents who contacted Customer Support in regard to not having the required documentation for proof of residency. Follow these instructions and the attached affidavit to submit the Proof of Residency Exemption affidavit.

This affidavit is intended to be completed by parents/guardians who reside with another
individual and do not have a utility bill, lease, or mortgage in their name to provide
during the application process.
- The parent will be provided the affidavit to complete.
- The parent will have the affidavit notarized.
- Once the affidavit is notarized, the parent will upload the affidavit along with an
appropriate utility bill, lease, or mortgage statement in the homeowner's/lease
holder’s name that was indicated on the affidavit. - The name on the proof of residency documentation needs to match the name
provided on number 2 of the affidavit document.
