This article explains the documents required to apply for and maintain eligibility in the Florida Empowerment Scholarship Program.

General Requirements
All applicants must provide documentation in the following areas:
✓ Student Identity
✓ Florida Residency
Income verification is not required but is recommended.
Providing income documentation helps determine where an application falls within the award prioritization process.
Student Identity Documentation
A Social Security Number must be entered for both the parent/guardian and student.
Additional Documentation Required
Students applying for the scholarship for the first time who are 5 or 6 years old on September 1 of the school year for which they are applying must upload one of the following:
✓ Florida birth certificate
✓ Certificate of live birth issued by a Florida hospital
✓ Valid, unexpired U.S. passport
Florida Residency Documentation
Parents or guardians must upload at least two (2) residency documents.
All documents must display:
✓ Parent or guardian name
✓ Current Florida address
Accepted Residency Documents
- Automobile insurance statement
- Florida driver's license or state-issued ID
- Health insurance statement
- Homeless verification or certification
- Homeowners or renters insurance policy
- Homestead or property tax statement/assessment
- Leave and Earnings Statement (LES)*
- Mortgage acceptance letter
- Mortgage statement
- Residential lease agreement
- Paystub
- Permanent Change of Station (PCS) orders*
- Proof of government benefits
- Residential internet bill
- Utility bill (electric, gas, or water)
* Applies to active-duty military families.

Income Verification
Income documentation is only used for award prioritization and is not used to determine scholarship eligibility.
Accepted Income Documents
- Most recent federal tax return (Form 1040, all pages)
- Most recent Florida tax return (Form 1040, all pages)
Active Duty Military Families
Additional Documentation Required
✓ PCS orders assigning the parent/guardian to a Florida duty station
Residency Verification for Families Living on Base
A base housing assignment letter may be used in place of a utility bill when the family resides on a military installation.
Important Note
Children of active-duty service members qualify as a priority group. Income verification documents are not required.
Military families must still submit:
✓ Student identity documentation
✓ Florida residency documentation
Law Enforcement Families
Additional Documentation Required
Submit one of the following:
✓ Agency-issued law enforcement ID or badge
✓ Employment letter on official agency letterhead confirming active sworn status and role title
✓ FDLE certification card
Important Note
Children of active sworn law enforcement officers qualify as a priority group. Income verification documents are not required.
Law enforcement families must still submit:
✓ Student identity documentation
✓ Florida residency documentation
✓ Enrollment documentation
Who Qualifies as a Law Enforcement Officer?
Under s. 943.10(1), F.S., a qualifying law enforcement officer must:
✓ Be elected, appointed, or employed full-time by the State of Florida, a Florida municipality, or political subdivision
✓ Be authorized to carry a firearm and make arrests
✓ Have primary responsibility for crime prevention, crime detection, or enforcement of criminal, traffic, or highway laws
Certified supervisory and command personnel also qualify when their duties include supervising, training, or managing law enforcement officers.
Eligible Roles
✓ Full-time sworn law enforcement officers
✓ Certified supervisory and command personnel
✓ School safety officers
✓ School resource officers
✓ Military police (through military eligibility)
Roles That Do Not Qualify
✕ Federal law enforcement officers
✕ Law enforcement officers from other states
✕ Auxiliary or reserve officers
✕ Correctional officers
✕ Correctional probation officers
✕ Security officers
✕ Firefighters
✕ Paramedics/EMTs
✕ Law enforcement support personnel
✕ Reserve deputies
Foster Care & Out-of-Home Care
Foster Care Documentation
Submit one of the following:
✓ Florida DCF placement order or court order
✓ Letter from the assigned DCF case manager
✓ Current case plan or service plan
Out-of-Home Care Documentation
Submit one of the following:
✓ Relative caregiver court order or safety plan
✓ Proof of Relative Caregiver Program (RCP) participation
Important Note
Children in foster care or out-of-home care qualify as a priority group. Income verification documents are not required.
Families must still submit:
✓ Student identity documentation
✓ Florida residency documentation
Unique Abilities (UA) Scholarship
Disability Documentation
Submit one of the following:
Individualized Education Program (IEP)
✓ IEP written in accordance with Florida State Board of Education rules
✓ IEP written in accordance with another state's requirements
Disability Diagnosis
A diagnosis from one of the following licensed professionals:
✓ Physician licensed under Chapter 458
✓ Physician licensed under Chapter 459
✓ Psychologist licensed under Chapter 490
✓ Physician licensed in another U.S. state, territory, the District of Columbia, or Puerto Rico
